      • Reminder: FinalForms will use the same parent/guardian email on file in PowerSchool
    4. Create your new FinalForms password and click confirm account.

    Students

    1. Students will use their district (SPS) email account to log into FinalForms
      • Reminder: Students new to the district (or participating as Athletics Only) should contact their school for log-in support

If you require any support during the process, review the FinalForms Parent Playbook or visit the FinalForms Support page.

Physicals are valid for 24 consecutive months.

Only the WIAA Physical Participation Form (linked above) is accepted.
