It is the mission of Chief Sealth International High School to provide the resources necessary to empower coaches and student-athletes to maximize their personal and athletic potential, and to be prepared socially, academically and physically to be positive contributors to the community at large, both now and in the future

How to Register For Athletics

Registration for Chief Sealth International High School Athletics is a FOUR (4) Step Process:

Step 1 : Parents/Guardians start the process by setting up an online account via FinalForms. Click the following link to a) create your account, b) add your student, and c) add your student’s email address.

Step 2 .  Students check their email for a message from FinalForms  with “new student account” as the subject. They read the email and sign the required forms for students athletes.

Step 3 .  Student Athletes pay ASB fees and/or any additional team fees on School Pay or in person with Mr. Patterson in the Fiscal Office. Students then provide receipt to Athletic Department. (NOTE: Due to COVID-19, we are currently not collecting ASB fees until we have confirmation that the sport season will occur)

STEP 4 .  Student, parent, or guardian uploads a copy of the student’s current physical form to FinalForms. Seattle Public Schools Physical Form

How do I sign Up? 

  1. Go to
  2. Click New Account under the Parent Icon.
  3. Type your Name, date of birth, and email; then click Register 
  4. Check your Email for a FinalForms Email, and click Confirm your account in the email text. Note: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, check your spam folder. If you still do not see the FinalForms email, please email
  5. Create your new FinalForms password and click Confirm Account.
  6. Click Register Student for your first child.

Enrollment/Cross-Enrollment Form-Athletics Only

This Out of District Athlete (ODA)/Athletics Only Form is required Students not enrolled in an SPS high school OR for SPS students enrolled in an alternative high school who wish to participate with their neighborhood school’s athletic programs Must Complete the Out of District Athlete (ODA)/Athletics Only Form


Insurance must be in effect according to Section II: Mandatory Athletic Insurance. If you use your private insurer, you need to give the name of the company providing coverage and a policy or employee number. Seattle School District Athletic Insurance is available. If interested please fill out the Student Insurance Enrollment Form (available in the Main Office at Chief Sealth) or enroll online Student Insurance Enrollment Online. It is your responsibility to be sure your insurance coverage is current.

Athletic Eligibility Requirements:

Chief Sealth is now moving to online registration for all sports using FinalForms, a secure online registration solution. Click the following link to complete your athletic paperwork online.

Participation Fee (ASB) and outstanding fines are due at the time of registration. The annual ASB Fee is $40, or $20 for recipients of Free/Reduced Lunch.

NOTE: ASB fees are waived for the 2020-2021 School Year

All students must have and maintain a 2.0 GPA or greater in 5/6 classes to participate (See the SPS Student Athletic Handbook for more detail).

Current health insurance is required for all athletes. Insurance information should be entered on page 2 of the Registration form and at the bottom of the trainer copy of the sport specific release form.  School year accident insurance plans are available for purchase arranged by Myers/Stevens/Toohey, pamphlets available in the main office.

Sports physicals are required prior to the first practice. *New Starting in 2020-2021* Physical examinations are good for 24 months from the date of the exam, and can be uploaded into FinalForms.

Refund Policy: The annual ASB Fee is non-refundable.

Transfer Students, private school, and students outside of the Chief Sealth International High School district should consult with the Chief Sealth Athletic Director for additional instructions for participation.

Sports Offered at Chief Sealth

Chief Sealth International High School offers 15 varsity sports and competes as part of the Metro League, one of the premier high school athletic leagues in the state.  In addition, many of our sports offer junior varsity and c-team competition depending upon annual turnout.  Please contact the Athletic Director or the individual coach for specifics on a particular team.
